Partnering to bring CMS approved Chronic Care Management solutions into Puerto Rico
SAN JUAN, Puerto Rico - The Consulting Lead (TCL) announced today that it has partnered with MCS Holding Company LLC (Medical Consulting Solutions “MCS”) to bring Chronic Care Management solutions into Puerto Rico.
In response to the challenge of caring for an aging US population and for other cohorts with special needs, the Centers for Medicare & Medicaid Services (CMS) has enacted a compelling package of financial incentives to help healthcare providers improve outcomes and reduce the cost of care for our neediest populations.
“We sought the opportunity to have this partnership with Medical Consulting Solutions to bring a feasible and holistic alternative to deal with Medicare and Medicaid funding in Puerto Rico,” said Gloria Fernandez, President of TCL.
According to CMS, a Chronic Care Management (CCM) service is considered as a critical component of primary care that contributes to better health and care for individuals.
For instance, by utilizing the appropriate technology to deliver a CMS-compliant Chronic Care Management program, patients with chronic conditions can receive better care beyond the walls of the physician practice resulting in better outcomes and fewer hospital admissions, while physicians receive new sources of income for managing “care-at-a-distance.”
"We contend that by implementing a Chronic Care Management program in Puerto Rico we can aid in improving the health & well-being of the patients, modernize the practice of care, reduce physician attrition, and create new career opportunities for the digital healthcare workforce of tomorrow, by fomenting a Community of Care (CoC),” stated Martin Nealon.
Such a CoC can be facilitated using modern technology with simple-to-use tools that promote patient engagement and chronic disease self-management, while linking patients, family members, physicians, nurses, and other caregivers.
“This collaborative approach to population health is a key to reducing costs across the healthcare system,” added Mr. Nealon.
